At the end of September, 2011, WebIZ received an upgrade! Some new features have been added and changes have been made, such as a new zip code lookup feature and the addition of the required NDC codes for vaccines. Click below to view a guide to the changes:

IMPORTANT INFORMATION!
The Nevada WebIZ Program has amended the existing regulations regarding the use of Nevada WebIZ, our statewide immunization registry. Effective January 28, 2010, prior consent is no longer needed to enter adult vaccinations- this means that patients of ALL ages who receive a vaccination must be entered in Nevada WebIZ.

  • Effective July 1, 2009, any person who administers any recommended immunization to a child shall report information concerning the child and the immunization provided to the Nevada State Health Division’s statewide immunization registry: Nevada WebIZ. Click below to download the text of both Nevada Revised Statute (NRS) 439.265 and the regulations adopted to carry out the new law.

It is stated in the regulations that vaccine lot details (lot#, expiration date, etc.) are required to be entered if available. Those offices currently entering their vaccine lot details in an Electronic Medical Record system (EMR) will not be required to enter the information in WebIZ.

If your office has never used the WebIZ Registry and you provide vaccinations, please download the Nevada WebIZ Enrollment form and Nevada WebIZ User Application/Agreement below to get started. Click on “WebIZ – Training” in the menu on the left to find a scheduled training in your area or to request an in-house training.

Forms
New office sites that have not previously used WebIZ should complete a Nevada WebIZ Enrollment form. This form contains instructions for completion.

Sites already using WebIZ and needing to establish additional user accounts should complete a User Application/Agreement for each new user.

 
Submitting Your Enrollment/Application Forms
All enrollment/application forms should be faxed (775-687-7596) or mailed to the address on the forms, “Attn: WebIZ Help Desk.”
