The Real Choice Systems Change Project is working to ensure and sustain a coordinated, family-focused, community-integrated system of care for all of Nevada's Children with Special Health Care Needs (ages 0-22 years).

About the Real Choice Systems Change Grant

The Real Choice Systems Change project is funded by a grant from the Centers for Medicare and Medicaid Services and focuses on improving access to and coordination of care for all Children with Special Health Care Needs, from birth to 22 years of age. The program is working with other State agencies like Medicaid, Welfare, the Office of Disability Services, Vocational Rehabilitation, and Early Intervention Services, along with health care providers, advocates, educators and, most importantly, families and youth with special health care needs to assess and improve the current system of care.

 

Focus areas include family inclusion, a more easily organized system of care, enhanced supports for a successful transition to adult life, and media outreach. The project also supports the Nevada Advisory Council for Children with Special Health Care Needs. This permanent 13-member councils membership consists of parents, providers, educators, and advocates. The Advisory Council serves to oversee project activities, as well as providing a forum for the needs and interests of Nevada�s Children with Special Health Care Needs. NeedsAssessment

Demonstration Projects

Another major goal of the Real Choice program is to establish demonstration projects to address the different concerns that have been introduced through the needs assessment. With the guidance of the Nevada Advisory Council for Children with Special Health Care Needs, the demonstration projects will be designed to address the different areas of need identified by the needs assessment for CSHCN. The demonstration projects will focus on a rural, northern urban, and southern urban region. The Nevada Advisory Council on Children with Special Health Care Needs has made recommendations to the Real Choice Systems Change Project on which key areas the Project's demonstration projects should address. The projects will begin in August of 2005 and this page will be updated as more information becomes available.

Media Campaign

The Real Choice program will be conducting various media campaigns to increase awareness of the various programs and activities that are available to families and their children with special health care needs. The campaigns will utilize a mix of radio and television ads in the form of public service announcements. As these announcements are aired more information will be provided on the media website.

 

The Real Choice Systems Change Project has partnered with the Nevada Broadcasters Association to air the various media information on Nevada's television and radio stations. Please view the media spots below!
