CSHCN Program |

This program assists in helping families
identify medical conditions in their children such as PKU,
galactosemia, maple syrup urine disease, etc. The program
also provides financial assistance for treatment to children
and their families who have been diagnosed with a medical
condition covered by the CSHCN program. |

Eligible Conditions |
The CSHCN Program provides limited
assistance, maximum $10,000.00 per year, only for eligible
conditions. Some severe, chronic or disabling disorders are
eligible, including but not limited to:

Central Nervous System defects
-
Cleft Palate and Craniofacial
Conditions
-
Convulsive disorders
-
Cystic Fibrosis
-
Eye conditions leading to loss of
vision (ordinary refractive errors are excluded)
-
Genito urinary system anomalies
-
Heart Disease - Rheumatic and
congenital
-
Hearing loss
-
Metabolic and Endocrine disorders
such as:
-
Orthopedic (bone) conditions such
as:
-
Reconstructive Surgery for
defects such as:
-
Pulmonary Conditions: Asthma -
BPD

Eligibility Criteria |
|
Individuals who meet the
following criteria may be eligible for coverage.

Resident of Nevada
-
A citizen of the United
States
-
A qualified alien, as defined
in 8 U.S.C. § 1641; or
-
An alien who is otherwise
eligible for participation in the program
pursuant to federal regulations regarding
the eligibility of aliens for public
assistance.

Meet established financial
criteria

Meet medical conditions
-
Meet age limits
-
Medical assistance - age 0-19
-
Metabolic- no age limit for
registry
ineligible

Ineligible Conditions |
The following are
a sampling of conditions not covered by the
CSHCN program. This sampling does not provide a
complete list of every condition not covered.

Acute infectious diseases.
-
Learning disabilities, mental retardation
and problems related to behavior.
-
Allergies.
-
Benign inflammatory conditions.
-
Blood and plasma, except for processing and
administrative fees.
-
Chronic sinusitis, except in cases of severe
respiratory impairment.
-
Custodial care.
-
Diagnostic or therapeutic procedures,
techniques, instrumentalities or agents
that:

Disorders of the immune system.
-
Initial acute care of accidents, poisoning
and violence.
-
Ordinary refractive errors.
-
Services for homemakers.
-
Transplant surgeries and drugs and supplies
directly related to the transplant.
-
The transportation of a client or a member
of his household, except that transportation
by ambulance is covered in unusual
circumstances if it is requested in advance
and there is documentation of the unusual
circumstances that created the need.

How to Apply |
|
To receive assistance you must
complete our application to determine your family’s
eligibility. Applications must be started as soon as
a need is known and submitted to an authorized

retroactive coverage for services obtained prior to

Once an Application Has
Been Completed |
|
Your application is submitted
to Family Health Services central office for review of
medical, financial, and administrative eligibility.
Applicant is advised of acceptance, denial, or pending
determination by a letter. Generally, determination is made
within 7 to 10 days of receiving the application. |

We ask that parent/legal guardian/client accept the
responsibility for assuring that this happens.
However, NRS 442.217 specifies that it is the
responsibility of the providers of care to notify
